The Chicago Health Atlas is a website for displaying aggregate health-related information on a map so that people can see the prevalence of specific health conditions in their area. It includes stats related to public health in Chicago (obtained from Chicago's open data portal), anonymized clinical diagnosis info (provided by CHITREC), and data about clinics, programs, and other resources (provided by Purple Binder, a Chicago-based company providing a directory of social services providers)

Created in 2005, CMAP is the official regional planning organization for the northeastern Illinois counties of Cook, DuPage, Kane, Kendall, Lake, McHenry, and Will.
